Phishing alert, unusual examples

Usually phishing alert triggers CVV, password related words. 

But there could be a rare example where the following sentence in Formbuilder app was triggering a phishing alert:

FIX: BROKEN_ATTACHMENT (original: https://help.powr.io/hc/article_attachments/4419597021207/mceclip0.png)

Phishing Attempt, App ID:- 8068151, Phishing Labels:- Question 10. Did you experience feelings of intense fear or helplessness after the event? *. If you think this is not supposed to happen, pleaseCLICK HEREto remove this app from phishing list.

It might be confusing to identify which word has triggered the alert, in this case it was:

"Helplessness"  word which contains SSN and SS (which stands for Social Security).
